En tant que personne impliquée dans la gestion, le management ou la coordination de réseaux de soins, de systèmes locaux de santé, d’un organisme médico-social, d’un hôpital, d’une maison de repos, d’une maison médicale, de services d’aides et soins à domicile... ces questions vous sont sans doute familières. Et si vous appreniez à identifier les meilleures stratégies à mettre en œuvre en fonction de la situation à laquelle vous faites face ?
Les enjeux et les réformes actuelles dans l’organisation des soins dans le monde mettent en avant le besoin d’améliorer l’intégration des soins en favorisant une meilleure collaboration interprofessionnelle ou inter-organisationnelle.
Ce cours vous propose une méthode et des stratégies pour analyser et améliorer l’intégration des soins au niveau des individus et de la population.
À partir de cas concrets, vous apprendrez comment analyser des situations de soins et à identifier les actions les mieux à même d’améliorer l’intégration des soins dans ces situations. Au niveau individuel, cela inclut des stratégies et outils d’évaluation de la situation, de formalisation de la planification du processus de soins ou de renforcement de la fonction de référent de soins.
Nous vous inviterons à appliquer cette démarche à des situations problématiques issues de votre propre contexte et à collaborer à leur analyse avec toute la communauté apprenante de ce cours. En choisissant la trajectoire certifiante, vous aurez l’occasion de mener une analyse complète de votre projet personnel avec l’équipe enseignante.

Prerequisites:
Connaissances de base en organisation des soins de santé, et/ou expérience professionnelle préalable dans ce domaine (dans les soins médicaux, les soins infirmiers, l'assistance sociale…).
Certains de ces prérequis peuvent être développés en suivant le cours en ligne « Approche systémique pour la gouvernance des systèmes de santé »

Au terme de ce cours, vous serez capables de

  • analyser les problèmes résultant d’un manque d’intégration dans une situation de soins individuelle
  • identifier les combinaisons possibles de stratégies pour améliorer l’intégration des soins au niveau individuel
  • analyser la gouvernance pour l’intégration des soins pour une population dans un territoire
