Manejo del enfermo semicrítico y crítico por COVID-19 (Coursera)

Después de realizar este curso serás capaz de conocer los elementos clave del abordaje del paciente crítico y semicrítico por COVID-19, basados en la evidencia científica disponible y que puedan ayudarte en la toma de decisiones clínicas. Curso dirigido a los profesionales de medicina y enfermería, con conocimientos básicos en la atención a pacientes críticos y, que debido a la crisis sanitaria, atienden en las áreas de críticos o semicríticos COVID-19 y/o a estos pacientes fuera de su ubicación habitual.

El programa docente se ha desarrollado desde el Máster Enfermo Crítico y Emergencias, programa de referencia en la formación de paciente crítico en la Facultad de Medicina y Ciencias de la Salud de la Universidad de Barcelona.

Syllabus

WEEK 1
¡Bienvenido/a! Programa formativo en el manejo del paciente semicrítico y crítico por COVID-19
Unidad 01-¿Qué es la COVID-19 ?
En esta unidad abordaremos los conceptos fundamentales sobre que es la COVID-19, como se produce su infección y profundizaremos sobre la fisiopatología asociada
Unidad 02 - ¿Cómo me protejo?
En esta unidad abordaremos los mecanismos de transmisión de la COVID-19, las medidas de protección, el riesgo de contagio en función de los diferentes dispositivos de oxigenoterapia y los diferentes dispositivos de nebulitzaciones. Puesta y retirada del EPI. Finalmente repasaremos los criterios de aislamiento y
cuarentena y su retirada.
Unidad 03- ¿Cómo se diagnostica?
En esta unidad abordaremos las manifestaciones clínicas, la utilidad de las técnicas diagnósticas y su interpretación. Cómo realizamos su diagnóstico clínico y la toma de decisiones asociadas. La presa de muestras PCR en esta patología. Finalizando con el repaso de insuficiencia respiratoria y la oxigenoterapia aplicada a la COVID-19.

WEEK 2
Unidad 04- Abordaje de la vía aérea
En esta unidad abordaremos las estrategias de abordaje de la vía aérea, la vía aérea difícil y la protección personal cuando la realizamos. Finalizaremos repasando la indicación y procedimiento de realización de traqueostomía y broncoscopia en pacientes con COVID-19.
Unidad 05- Sedación y analgesia
En esta unidad abordaremos las estrategias de sedoanalgesia y relajación en el paciente crítico con COVID-19 y su monitorización así como el diagnóstico del delirium y su abordaje.
Unidad 06- Medidas de soporte respiratorio
En esta unidad abordaremos las medidas de soporte respiratorio desde la oxigenoterapia de alto flujo, la ventilación mecánica no invasiva, las terapias inhaladas y su monitorización y respuesta, así como también la fisioterapia respiratoria.

WEEK 3
Unidad 07-Ventilación mecánica invasiva
En esta unidad abordaremos la ventilación mecánica invasiva, sus principios y parametrización, particularidades de los pacientes con COVID-19, las asincronías y el drive respiratorio, las señales de alarma y finalmente su retirada (weaning).
Unidad 08- Abordaje de la hipoxemia refractaria
En esta unidad hablaremos del abordaje de la hipoxemia refractaria: la optimización de la PEEP, el decúbito prono y las terapias de soporte vital extracorpóreo ECMO y ECCO2R.
