To provide an up-to-date overview of the disease, the Institut Pasteur has created a MOOC dedicated to Tuberculosis. This course will provide a knowledge base that goes from the basics of TB to the current state of knowledge on various aspects of tuberculosis: the clinical picture, epidemiology, pediatric TB, factors affecting susceptibility to the disease, including HIV, diabetes and human genetics, immunology, vaccines, diagnosis, current and future ways to detect drug resistance, and how to treat drug sensitive and multi-drug resistant strains.

Tuberculosis has been a scourge of humanity for hundreds of years. In the 18th and 19th centuries, tuberculosis was called the white plague and in some countries in Europe and the US was responsible for 20% or more of all deaths. Today, out of all infectious agents, tuberculosis remains the biggest killer, surpassing even HIV and malaria. In 2015, according to WHO, there were nearly 10.4 million new cases of tuberculosis and around 1.8 million people died from the disease, making it one of the top 10 causes of deaths worldwide. Although there are plans to eliminate it within the next couple of decades, the incidence is currently only falling by about 1.5% per year. Part of the problem is the nature of the disease, which takes months to develop and requires months of multi-drug chemotherapy to cure. It can also stay latent for decades and then emerge to cause serious illness. Another problem, this one man-made, is the drug resistance and multi-drug resistance that make TB much harder to cure.

To provide an up-to-date overview of the disease, the Institut Pasteur has created a MOOC dedicated to Tuberculosis. This course will provide a knowledge base that goes from the basics of TB to the current state of knowledge on various aspects of tuberculosis: the clinical picture, epidemiology, pediatric TB, factors affecting susceptibility to the disease, including HIV, diabetes and human genetics, immunology, vaccines, diagnosis, current and future ways to detect drug resistance, and how to treat drug sensitive and multi-drug resistant strains. It will also discuss clinically important non-tuberculous mycobacterial infections and how to diagnose and treat them. It will present recent advances in molecular techniques for diagnosis and drug sensitivity testing, including Whole Genome Sequencing, and the new, shorter and more effective treatment regimens to cure Multi-Drug Resistant TB.
The course aims at providing a broad overview of the current state of knowledge about tuberculosis for physicians, nurses and other health and public health workers, microbiologists, pharmacists, researchers and students. It will serve as a valuable update for people working with tuberculosis, a thorough introduction for those who are new to the field and a great opportunity for all those who want to broaden their understanding of the disease.
La tuberculose est un fléau de santé publique depuis des centaines d’année. Aux 18e et 19e siècles, on la qualifiait de peste blanche et dans certains pays en Europe et aux Etats-Unis, la tuberculose était responsable de 20% de tous les décès. Aujourd’hui, elle reste la maladie infectieuse la plus mortelle, devant le SIDA et le paludisme. En 2018, selon les chiffres de l’OMS, 10 millions de personnes ont contracté cette maladie et 1,5 million en sont mortes, faisant de la tuberculose l’une des dix premières causes de mortalité dans le monde. Malgré des plans d’élimination de la tuberculose dans les deux prochaines décennies, l’incidence annuelle ne diminue que de 1,5% par an. Le problème provient en partie de la nature même de la maladie, qui se développe lentement pendant des mois et dont le traitement nécessite plusieurs mois d’une association de plusieurs médicaments. La tuberculose peut aussi rester latente pendant des décennies puis émerger soudain et provoquer une maladie grave. Une autre difficulté, liée celle-ci à l’homme, provient de l’apparition de résistances au traitement, voire de multi-résistances, qui rendent la guérison beaucoup plus difficile.
L’Institut Pasteur a entrepris de réaliser ce MOOC pour faire le point sur la tuberculose et ses derniers développements. Ce MOOC couvrira pour les participants des sujets très variés, allant des connaissances de base jusqu’aux dernières découvertes sur les aspects cliniques, l’épidémiologie, la tuberculose infantile, les facteurs influant sur la susceptibilité à la maladie (infection par le VIH, diabète, facteurs génétiques), la réponse immune à l’infection, les vaccins, les méthodes de détection de la résistance et le développement de traitements efficaces contre la tuberculose (y compris multirésistante. Ce MOOC évoquera aussi les cas d’autres infections à mycobactéries (en dehors de la tuberculose), la manière de les détecter et de les traiter. Ce MOOC présentera également les avancées récentes des techniques moléculaires de diagnostic et de test de sensibilité aux médicaments, y compris le séquençage du génome complet et les nouveaux traitements contre la tuberculose multirésistante.
Ce MOOC vise à apporter un panorama large sur la tuberculose, s’appuyant sur les dernières découvertes des laboratoires, aux professionnels de santé et de santé publique, aux microbiologistes, aux pharmaciens, aux chercheurs et aux étudiants en sciences de la vie. Ce MOOC se propose également de servir de mise à jour pour les médecins, les infirmières et les travailleurs sociaux travaillant sur la tuberculose ou dans la prise en charge des malades, mais également d’introduction solide pour ceux qui ne sont pas familiarisés avec la maladie, souhaitent élargir leurs connaissances et améliorer leur compréhension.

Format
This MOOC is organized in 6 weeks. Each week is composed of 4 to 6 sequences. In each sequence, you find a 10 minute video and two multiple choice questions (MCQ) to help students check their understanding. The videos are in English subtitled in French and in English. A certification exam (in English or in French) is accessible only for students registered to the “qualifying path”.
Ce MOOC est composé de 6 semaines d’enseignement. Chaque semaine comporte 3 à 6 séquences. Chaque séquence se compose d’une vidéo d’environ 10 minutes et de deux QCM afin que les étudiants testent leurs connaissances. Les vidéos sont en anglais sous-titrées en français et en anglais. A la fin de chaque chapitre, des questions à choix multiple en anglais vous seront posées. Un examen de certification (en français ou en anglais) est proposé aux étudiants inscrits au parcours qualifiant.

Requirements /Prérequis
This MOOC is aimed at people interested in Tuberculosis: microbiologists, researchers in infectious diseases, biologists, physicians, epidemiologists, public health workers and patient education, as well as students in any of these areas.
A bachelor of science, a degree in a health related field or a good scientific background is recommended to be able to participate in this MOOC.
Ce MOOC s'adresse à tous ceux qui souhaitent mieux comprendre les données actuelles et les challenges futurs de la tuberculose : microbiologistes, chercheur en infectiologie, médecins internes et infectiologues, épidémiologistes, médecins de santé publique et responsables institutionnels.
Il est conseillé de disposer d’une licence de sciences, d’un diplôme dans le domaine de la sante ou plus généralement de connaissances en sciences pour suivre ce MOOC.
